Market growth is increasingly tied to wider treatment of moderate-to-severe vascular calcification, particularly in complex PCI and peripheral artery interventions.
Coronary IVL was the largest application segment, generating approximately USD 816.8 million in 2025, or about 70% of global market revenue. The segment is projected to approach USD 2.38 billion by 2032 as IVL becomes more routinely incorporated into treatment of heavily calcified coronary lesions.
Peripheral IVL generated approximately USD 344.6 million in 2025 and is projected to exceed USD 1.0 billion by 2032. Within peripheral applications, femoropopliteal disease represented the largest segment at approximately USD 170 million, followed by iliac and below-the-knee interventions.
IVL adoption is also being supported by broader use of intravascular imaging, which allows physicians to identify calcium thickness, distribution and severity before stent deployment. This is increasing the role of planned calcium modification in complex coronary procedures.
The competitive landscape is expanding beyond Shockwave Medical, now part of Johnson & Johnson. Boston Scientific has strengthened its IVL position through Bolt Medical, Abbott is advancing its own IVL platform, and Elixir Medical is developing an alternative calcium-modification approach. Competition is increasingly focused on catheter deliverability, lesion crossing, procedural simplicity, clinical evidence and cost.
The United States remains the largest IVL market, supported by high complex-PCI activity, reimbursement and early adoption. Germany is an important Western European market because of its high PCI intensity and increasing use of intracoronary imaging, while Japan represents a major Asia-Pacific opportunity due to its large PCI base and extensive use of IVUS and OCT.
Through 2032, market expansion is expected to depend increasingly on deeper treatment penetration within existing calcified coronary and peripheral procedures, broader access to difficult lesions, improved reimbursement and adoption of next-generation IVL catheters.
